Living an Everyday Life is a great concept, but how does a person with a disability get from their person-centered plan to their ISP to directing their own services (or at least identifying what they want!)? Connect the dots with us as we offer examples of putting a plan into action with this workshop.
- Participants will understand the importance of person-centered planning
- Participants will see the impact of a person-centered plan on the ISP
- Participants will see how a Person-centered ISP can influence services and supports under participant direction
